2nd Home Family Childcare
              Admission Agreement


Basic Services   
2nd Home Childcare is licensed family childcare serving children infancy through
school age.  The hours of operation are 8:00am until 5:30pm, Monday through Friday,
with the exception of stated holidays or center closings.  I offer care full time and
part time care.  
   
Tuition & Fees     
Full tuition is due on the first of each month.  If payment has not been received by the
fifth of the month, your child will not be admitted unless prior written agreements
have made. A $20.00 (a day) late payment fee will be charged on all tuition received
after the fifth of the month.  In addition, a $30.00 fee will be charged if a check is
returned.
No deductions or refunds will be made for absences or holidays
I do not allow makeup’s for missed days
FULL TUITION IS DUE FOR ALL HOLIDAYS, MISSED DAYS, AND CHILDCARE
CLOSURES..   

Parents will be notified of any rate changes at least 30 calendar days in advance.

Registration fees are due upon enrollment for each session and are nonrefundable.

Late fees of $1.00 a minute may be assessed for parents picking up children later than
the scheduled time chosen at registration.  

Withdrawal      
Parents are required to submit two week’s written notice before withdrawing a child
from the program.  If no notice is given parents will be responsible for payment of two
week’s tuition whether or not their child is in attendance.

2nd Home Childcare reserves the right to terminate the admission agreement at any
time for any reason.  Unused tuition will be refunded if I choose to terminate the
agreement.  For example, a termination could result if the program does not meet your
child’s need or if negligence is demonstrated in tuition payments.

Illness Policy  
A medical assessment form, including proof that immunizations are up-to-date, must
be on file within 30 days of the child’s enrollment.  For emergency purposes, the
consent for Medical Treatment and the Emergency and Identification forms must be
completed before the child’s first day.

Rights of licensing Agency
The Department of Social Services and/or licensing agency shall have the authority to
interview children or staff and to inspect and audit children’s or facility records
without prior consent.  The Department or Licensing agency shall have the authority
to observe the physical condition of the child (ren), including condition which could
indicate abuse, neglect, or inappropriate placement, and to have a licensed medical
professional physically examine the child (ren).  {Section 101195(b), (c).}

I have received, understand, and agree to comply with the 2nd Home Childcare’s
policies as described in the Admission Agreement and the Parent’s Handbook.  If
there is any default in this agreement, the prevailing party shall be entitled to attorney’
s fees as well as the cost involved in collections.
